Child Identification,
Protection, and Safety Program |

Children are our most valuable resources and deserve our utmost interest and
protection. The National Center for Missing and Exploited Children estimates
that on average - more than 2,000 children are reported missing every day.
|
|
|
With the full support of the Carson City Council, the Child Identification,
Protection and Safety Program was established in July, 2008. This program
provides parents with identification cards that can be used to provide vital
information to law enforcement in any incidents involving their children. In
addition, the system is available for any adults who wish to take advantage
of its benefits.

The City of Carson is offering a downloadable identification form on the
city’s website. To proceed, please download and
complete the PDF online form. No record is maintained by the City of Carson.
You may also save the form to your computer and complete it at your
convenience. |

Child/Adult Identification completion instructions: |

Enter the required information next to the indicated
fields. |

With your computer mouse, left click the photo area to
insert a digital photo. |
| 3) |
Print the completed form. |
| 4) |
Using your own ink pad, place the thumb prints in the
designated areas. |
| 5) |
The top portion is to be retained at home. The cutout
portions are wallet-sized identification cards for parents or guardians
to carry with them at all times. |

The identification card should be updated annually so
that the information on the card is current and accurate. |
